Handovers is the process of transferring patient information from one shift to the next shift until the patient is no longer in the unit. The nursing change of shift report or handover is a communication that occurs between two shifts of nurses whereby the specific purpose is to communicate information about patients under the care of nurses (Lamond, 2000). Transfer of patient information is through oral handovers. Patient information resources are records that have been documented in patient files. This information will be transferred to the next shift nurse during the handover and will be recorded in the task hand book record.

duty. Recognise that effective handovers involve two
simultaneous dimensions of communication: the interactional and the
informational. (Eggins S, Slade D. 2015).  According to Miller (1998), there are several studies
showing how the handover and four main handover are: Verbal handover, Tape
recorded handover, Bedside handover, written handover. But handover pattern
practiced in hospitals is three in one, initiated from one patient to another,
verbal and verbal delivery of information while the nurse who takes over will
write the information obtained into the handover record book.

The purpose of this record is to refer to the nurse itself and as evidence of what has been conveyed to the nurse of the shift and may be evidence of a case involving the court. According by Bhabra et. Al (2007), an evaluation of handover practice showed that only 33% of data transferred verbally was retained, which retention improved to 92% when verbal handover was supplemented by note-taking.
